Information for GPs

Admitting a patient:

Phone our Hospital Co-ordinator on (03) 9474 8900 and provide the following details:

• Address
• Any risk history or potential risk factors
• Contact numbers
• Current regime
• Date of birth
• Health insurance details
• Outline of current issues
• Patient's name
• Previous treatments.

To ensure the patient has adequate cover, the co-ordinator will take the patients details, ensure a health fund check is done and will contact you to discuss the admission.
